How To Get Started

Where To Start:

You must obtain a copy of your most recent medical records (at least 2 visits within the last 5 years), from your primary doctor. The records must contain your qualifying condition. If you do not already have a copy of your medical records we can help you obtain them.

Once your records are received by our office, the clinic director will put together your chart and then the doctor reviews them.

Then...

Once your records are reviewed, our staff will call you to schedule an appointment to see the doctor during a clinic. Our clinics are usually held once a week sometimes twice a week.

What to Expect...

Clinic Day:

You will see the doctor during a scheduled appointment time on a clinic day. Please arrive 15 minutes early for your appointment so that you can fill out the necessary paperwork (4 forms) before seeing the doctor.

NOW ACCEPTING

Now What...?

Once the doctor has seen you and signed your card, the state REQUIRES you to register with them.

There is a registration fee which is seperate from the doctor's fee.

The fee is $100.00 unless you can show proof of Low Income.

Proof of low income is...

Oregon Health Plan(OHP),

Food Stamps, or SSI.

If you can show proof of one of these, the low income fee is $20.00.
